Freedom in Christ Ministries (FICM) is seeking to stay connected with those around the country who are starting and growing Community Freedom Ministries (CFM) through their home churches.

The applicant has applied to become a part of this strategy to stay connected through our CFM NETWORK. He/she has also been through an interview with some of our FICM personnel, helping us get to know him/her better.

The last phase of the CFM NETWORK application process is for you as a church leader to give us your feedback regarding the applicant. We would appreciate your taking a few minutes to answer the following questions to the best of your ability. These answers will be kept strictly confidential.

Once this questionnaire is received in our office, we will make our final decision regarding the applicant's acceptance to the CFM NETWORK.

FICM will provide ongoing assistance to the applicant (should he or she be accepted as part of the CFM NETWORK) in the form of discounted resources, training events and personal mentoring. FICM is not, however, able to maintain the level of supervision over CFMAs that church leaders can.
